You are invited to come to the reception, with an ID card and following documents:
Whatever is your social cover (general, not salaried, agricultural worker) to whom you are affiliated:

A coverage of your complementary health insurance, specifying the expenses which it covers.
If you benefit from the free universal healthcare:

Your certificate of opening of rights.
If you are a beneficiary of the medical aid of state:

Certificate of the opened rights.
If you are a war pensioner:

Your certificate of care.
If you are a victim of an accident at work:

The document of works’ accident given by your employer and healthcare card.
If your intervention is subjected to a preliminary agreement of the health insurance fund:

The document of preliminary agreement.
Also bring all the medical documents in your ownership:

Letter of your GP.

Card of blood group.
-
X-rays, results of analysis, electrocardiogram.

Prescriptions indicating the current treatments.
In case of plastic surgery, the totality of the expenses of hospitalization is chargeable to the patient.
ADMISSION
In conventional hospitalization, the admissions take place between 3 pm and 5 pm.
Admissions to the service of ambulant surgery take place at 7 am.
ROOMS
If you wish a single room, your request will be satisfied in the extent of our possibilities the day of your admission.
ANONYMITY
If you wish not to reveal your presence in the establishment, indicate it to the person in charge of the units of care and the necessity will be made to protect your anonymity.
DRUGS
If you have a particular treatment delivered by your GP, you have to bring the medicines prescribed and the prescription of your doctor. You’ll have to inform the doctor of the establishment and the nurses about it. Take no one other drug than those prescribed by the practitioners of the private hospital.
PERSONAL THINGS
You have to think to bring:

A toilet bag: soap, toothpaste, toothbrush, razor, tissues.

Wash towels and gloves.

A pyjama or a nightdress and a housecoat.

Slippers.
The private hospital does not operate the cleaning of the linen.
VALUABLES
We deeply recommend you not to keep jewels or valuables in your room. As possible, we ask you to entrust them to a member of your family. Nevertheless, a safe is at your disposal to the administrative department. You can deposit your values there; a depository receipt will be given to you.
According to the law of July 6th, 1992, the establishment cannot be considered responsible for the stealing, the damage or the loss of valuables not deposited in the safe.
