According to the legal measures relative to the access to the personal information that an establishment of health is in possession of (law of March 4th, 2002), you can access to your medical file, with two ways:
Or: Directly by a mail sent to the Director of the establishment with a supporting evidence of identity.
Or: Through a doctor appointed by you in writing, with a supporting evidence of your identity.
